Please fill out the following Client Disclosure and Client Intake forms prior to your first session, and either print and bring them with you to your appointment or email them to me prior to your appointment (preferred). If I am seeing your minor child for a session, the Minor Consent Form along with a Client Intake form for the child must be filled out, and a parent must sign the Client Disclosure form as well. There will be additional forms provided for you if you are scheduling an intensive with me.
